Brow Lift (Forehead Lift)

Overview

A brow lift — also known as a forehead lift — is a surgical procedure that elevates drooping brows, smooths forehead wrinkles, and opens the upper face to restore a refreshed, alert, and naturally youthful appearance. At The Ferguson Clinic in Honolulu, Dr. John C. Ferguson, MD, FACS, performs brow lift surgery using endoscopic and direct techniques refined over more than 25 years of cosmetic and facial plastic surgery practice. As a quintuple board-certified surgeon with dual certification in both Facial Plastics & Reconstructive Surgery and Otolaryngology — Head & Neck Surgery, Dr. Ferguson approaches the forehead and brow with a structural understanding of the muscles, nerves, and tissue planes that determine brow position and expression.

Brow ptosis — the descent of the eyebrows that occurs with aging — is one of the most underdiagnosed contributors to a tired, heavy, or angry appearance. Many patients who think they need upper eyelid surgery actually need a brow lift, or a combination of both. Dr. Ferguson’s expertise in both procedures allows him to make that distinction precisely and recommend the approach that will produce the most natural, lasting result.

Am I a Good Candidate?

You may be a good candidate for a brow lift if your eyebrows have dropped below the brow bone, creating a heavy or hooded appearance; you have deep horizontal forehead creases or vertical frown lines (glabellar lines) that are present even at rest; your brow position gives you a perpetually tired, sad, or angry expression; or you feel that your upper eyelid heaviness is caused by brow descent rather than (or in addition to) excess eyelid skin.

Ideal candidates are in good general health, have realistic expectations, and do not smoke. During consultation in Honolulu, Dr. Ferguson performs a detailed assessment of brow position, forehead muscle activity, hairline position, and upper eyelid anatomy to determine whether a brow lift alone, blepharoplasty alone, or a combination procedure is most appropriate.

The Procedure — What to Expect

Dr. Ferguson performs brow lift surgery at his AAAHC-accredited surgical facility in Honolulu, typically under general anesthesia or IV sedation. The procedure takes approximately one to two hours depending on technique.

Endoscopic brow lift is Dr. Ferguson’s most common approach. Three to five small incisions are hidden behind the hairline. A thin endoscope (camera) provides magnified visualization while specialized instruments release the tissues that pull the brow downward. The brow is elevated to its ideal position and secured with absorbable fixation devices. This technique produces minimal scarring, less swelling, and a faster recovery than traditional open brow lifts.

Direct brow lift may be appropriate for male patients with deep forehead creases or receding hairlines, where a direct incision above the brow or within a prominent forehead crease can be concealed within the natural skin texture.

Temporal (lateral) brow lift addresses only the outer portion of the brow — the area most affected by gravity — and is frequently combined with upper blepharoplasty for a comprehensive upper face rejuvenation.

Recovery and Results

Days 1–3: Mild swelling and tightness across the forehead. Some patients experience temporary numbness or tingling behind the incisions. Cold compresses and head elevation minimize swelling. Prescribed pain medication keeps discomfort well-controlled.

Week 1: Bruising, if present, migrates downward toward the eyes and resolves over seven to ten days. Sutures or staples are removed at seven to ten days. Most patients feel comfortable returning to desk work and light activities within a week.

Weeks 2–4: Swelling continues to resolve. Sensation returns progressively. Light exercise resumes at two to three weeks; strenuous exercise at four weeks.

Months 1–3: Scars within the hairline fade and become virtually undetectable. The brow settles into its final position. Patients see a refreshed, open appearance that looks natural — not surprised or overdone.

Frequently Asked Questions

Will a brow lift make me look surprised?
Not when performed by an experienced facial plastic surgeon. Dr. Ferguson elevates the brow to its anatomically appropriate position — restoring where the brow used to sit rather than pulling it unnaturally high. The goal is a refreshed, rested appearance with natural expression preserved.

What is the difference between a brow lift and Botox for the forehead?
Botox relaxes the muscles that create dynamic forehead wrinkles and can provide a mild brow elevation, but it cannot address structural brow descent or significant skin excess. A brow lift physically repositions the brow and removes or redistributes tissue for a permanent correction. Many patients benefit from both — a brow lift for structural correction and Botox maintenance afterward.

Do I need a brow lift or eyelid surgery?
This is one of the most important distinctions in upper face rejuvenation. Many patients attribute heaviness in the upper eye area to excess eyelid skin when the real cause is brow descent pushing tissue downward. Dr. Ferguson evaluates both the brow and the eyelid during consultation to recommend the procedure — or combination — that addresses the actual anatomy.

Will a brow lift affect my hairline?
Endoscopic brow lift incisions are hidden within the hair-bearing scalp and do not alter the hairline position. For patients with high foreheads, a hairline brow lift (trichophytic approach) can lower the hairline while simultaneously lifting the brow.

How long does a brow lift last?
A brow lift typically provides seven to twelve years of significant improvement. Gravity and aging continue after surgery, but patients consistently look younger than they would have without the procedure. Results can be extended with non-surgical maintenance such as Botox and skin care.

Why Choose Dr. Ferguson for a Brow Lift in Honolulu

The forehead and brow are part of the face — and the face is where Dr. Ferguson has spent his entire career. His dual board certifications in Facial Plastics & Reconstructive Surgery and Otolaryngology — Head & Neck Surgery mean he understands the deeper anatomy of the forehead at a level that general cosmetic surgeons simply cannot match. The frontal branch of the facial nerve, the corrugator and procerus muscles, the periosteum — these structures determine whether a brow lift produces a natural result or an unnatural one. Dr. Ferguson’s training ensures the former.
